• Job Demands – Detailed description of physical and environmental demands of each unique job.
• Medical History – Thorough online questionnaire customized to match the job demands.
• Worker and PCP Interviews – Follow-up telephone interviews by a HealthForce physician with the worker and their PCP as necessary.
• Examinations and Testing – Additional examination and testing prescribed by a HealthForce physician as necessary.
• Clearance Letter – Written opinion by HealthForce physician indicating whether the worker has the capacity to safely meet the demands of the job and specifying any required limitations or restrictions so that the employer can consider whether accommodations are reasonable.
• Medical Records – Record of job demands, questionnaire, examination and test results, provider analysis, and clearance letter all held by custodian for the benefit of the employer and made available to the worker.
