• Job Demands – Detailed description of physical and environmental demands of each unique job, including exposures.
• Medical History – Detailed medical and occupational health history questionnaires, including OSHA prescribed question sets.
• Examinations and Testing – Physical exams and testing, including laboratory analysis, for baseline, periodic, post-exposure and exit evaluations.
• Worker and PCP Interviews – Follow-up telephone interviews by a HealthForce physician with the worker and their PCP as necessary.
• Clearance Letter – Written opinion by a physician indicating that the worker does not have any detected medical conditions which would place them at increased risk of material health impairment from hazardous material exposures or specifying any limitations or restrictions required due to the employee’s health condition.
• Medical Records – Record of job demands, questionnaire, examination and test results, provider analysis, and clearance letter all held by custodian for the benefit of the employer and made available to the worker.
• Expiration Tracking – Continuous reporting on expiring clearances.
