HOW TO PREPARE FOR YOUR
DOT Examination

WHAT IS A DOT MEDICAL EXAM?
A Department of Transportation (DOT) Medical Exam is required for commercial drivers and other safety-sensitive workers to ensure they are physically and mentally fit to operate a commercial motor vehicle safely. Click here to learn more about driver medical requirements.
WHAT YOU NEED TO DO

BRING REQUIRED ITEMS

The Service Authorization, either printed or digital copy

Your valid photo ID
-
A complete list of all medications and dosages

For any of the following medical conditions that apply to you:

Glasses, contacts, or hearing aids

Monocular Vision: please complete this form
-
CPAP: compliance report with one year of data, 90 days minimum

Heart-related issues: a cardiologist letter outlining your medical history and ability to drive a commercial motor vehicle stress test or echocardiogram from last 1-2 years

Coumadin (Warfarin): recent INR Letter from your doctor

Diabetes: Hemoglobin A1C lab result from the last 3 months

Take Insulin? Ask your personal doctor to complete this form

Other medical conditions (including mental health): treating provider letter stating you are cleared to operate a commercial motor vehicle

BEFORE THE EXAM

Take prescribed medications as usual, unless advised otherwise by your doctor

Get a good night’s sleep before your exam
-
Stay hydrated and avoid excessive caffeine or salty foods before the exam as they may affect your blood pressure

DURING THE EXAM
-
A review of your medical history and medications
-
A physical examination, including vision, hearing, blood pressure, and urinalysis

Additional tests, if required based on your health history
WHAT HAPPENS AFTER THE EXAM?

If you meet DOT health requirements, you will receive a Medical Examiner’s Certificate (DOT Medical Card).

If additional medical evaluation is needed, you may be required to provide further documentation or follow-up testing.

If you do not meet DOT standards, the medical examiner will discuss your options.
