If a HealthForce Physician needs information from your personal healthcare provider, you will receive a PCP Letter request by e-mail.

WHAT IS A PCP LETTER?
The PCP Letter is addressed to your personal healthcare provider explaining what information the HealthForce Physician needs to help move your case forward.

WHAT YOU NEED TO DO
1   Log into www.MyHealthForce.com.
2   Click the Documents link in your case.
3   View and download the PCP Letter.
4   Send the letter to your personal healthcare provider, requesting that they respond to HealthForce ASAP.
5   Your personal healthcare provider must respond directly to HealthForce.
6   You will receive a notification when your case is closed. Log into www.MyHealthForce.com to confirm that your

      healthcare provider responded to HealthForce. Click the Documents link on your Home Page.

The letter cannot be e-mailed to you because it is not a secure way to transmit your private health information.
